You can notify the Ontario government of a change of address for your Driver’s License, Vehicle License and OHIP together at www.addresschange.gov.on.ca.
If you move, under the Highway Traffic Act, you must notify the ministry within six days of changing your residential address. The address must contain a street number, street name, apartment number (if applicable), city and postal code OR a lot, concession, township and rural route, city and postal code. Use the Online Form or take the change of information form that comes with your licence to a ServiceOntario Kiosk, Driver & Vehicle Licence Issuing Office, or mail it or a letter, stating your driver's licence number, your old and new addresses to:
Ministry of Transportation
Licence Renewal Section
P.O. Box 9200
Kingston, ON K7L 5K4

• OHIP

There are four ways to update your address :
  1. Use Ontario Online Services to send your change of address electronically.
  2. Go to any ServiceOntario Kiosk. They are similar to automated teller machines and provide extended hours of service seven days a week. You will need your health card and the health cards of all members of your family as well as your new postal code.
  3. Obtain a Change of Address (form 1057-82) and return it to the ministry by mail. Forms are available on Forms Online or from your local OHIP office.
  4. Send a letter or visit your local OHIP office or Outreach site. You must include your name, health number, telephone number, current address, and new address including postal code.

If you have a photo health card and update your residential address, you will receive a new health card with your new address. If your photo health card has expired, you must visit an OHIP office in person.

If you have a red and white health card, you will continue to use it but the ministry database will be updated with your change of address information.
